Dartmouth-Hitchcock Clinic in Lebanon, NH is seeking a board certified Family Medicine faculty member to join and help launch our new Patient Centered Medical Home dedicated to employees and their adult dependents. Opportunities for candidates interested in a full time appointment, which might include work in other D-H practices, educational programs or research may be explored.
Dartmouth-Hitchcock Primary Care is a dynamic group with a well-established clinical quality improvement focus, moving to a new integrated D-H Primary Care Center with a focus on outcomes and population health. We are actively engaged in re-designing how we deliver primary care and looking for faculty with enthusiasm and interests in outcomes research, population health, new models of care delivery focused on an employed population, and innovative teaching approaches. The successful candidate will provide team based clinical care and will participate actively in teaching of medical students and residents.
This position includes a faculty appointment in the Department of Community and Family Medicine at the Geisel School of Medicine at a rank commensurate with experience.
Dartmouth- Hitchcock is located in the Upper Valley of New Hampshire and Vermont, a vibrant community offering excellent schools and outstanding quality of life in a beautiful rural environment.
Desired background for the successful candidate includes three to five years of clinical practice in primary care, preferably in a Patient Centered Medical Home; keen interest in preventive and lifestyle medicine; and demonstrated leadership.
