Program Navigator-Endocrinology

Provides support for patients looking to have care within the Transgender Program. Requires a strong patient advocate to optimize the patient experience for those seeking care specific to transgender care. Supports an interdisciplinary team across multiple sections by serving as the liaison among the patient, healthcare providers and the healthcare system.


  1. Identifies transgender patients who require extra help in navigating the healthcare system.
  2. Under the direction of the Medical Director of the Transgender Program, oversees the development of a clinic structure that includes multiple specialties and locations.
  3. Facilitates communication and coordinates gender health services across multiple specialties and locations, including for gender affirming surgery until time of surgery.
  4. Manages and coordinates patient centered care by acting as the liaison between patients and their multidisciplinary care team both within and external to DH.
  5. Utilizes innovative strategies to advocate for patient needs and negotiates complex systems to remove barriers and limitations in accessing health care.
  6. Organizes and distributes materials and patient education information for the clinic, patients and their families.
  7. Participates in preparing and providing education as needed to internal service providers.
  8. Partners with the Diversity, Equity and Inclusion Program as needed.
  9. Collects and evaluates data as needed to measure the success of the Transgender program and reports results to leaders.
  10. Performs other duties as required or assigned.
  11. On site daily with potential opportunity for occasional remote work.


High school graduate required.
Bachelor's preferred


Estimated Salary: $20 to $28 per hour based on qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
